Personalized medical

billing support 

 

  • Starting when you call to make your first appointment, we will work together to connect you with our medical billing partner for verification of insurance benefits, pre-authorization, and handling of your claims. 

​

  • We will need your insurance card(s) and sleep study to get started.

Accepted payment methods

​

  • Credit Card (Amex, VISA, MasterCard, Discover)

  • Care Credit

  • HSA and FSA 

  • Cash/Check/Debit

What to bring to your first appointment

​

  • Driver's License or State ID

  • Medical Insurance Card (including the policyholder’s date of birth)

  • Medicare card and any supplemental policy cards (if applicable)

  • Copy of your most recent sleep study or studies (these can also be faxed 

  • or emailed) if you have had this done

  • Prescription for Oral Appliance Therapy (these can also be faxed or emailed) if you have this

  • Physician Referral (these can also be faxed or emailed) if you have this

Insurance Policies

  • Regardless of the estimated insurance coverage, payment is due in full at the time of your appointment unless a payment plan is approved in advance. 

​​

  • Payment plans may be available upon request.

​

  • All insurance plans are not the same and do not cover the same services. In the event your insurance determines a service is not covered, you will be responsible for the complete charge.

 

  • We are an out-of-network provider. Your insurance policy is a contract between you and your insurance company. As a courtesy, we will file your insurance claim for you.  

​

  • If you receive payment from your insurance carrier when the payment should have come directly to our office, we request that you bring the check and explanation of benefits or attached paperwork into this office. Failure to do so will result in collection action.

​

  • Services rendered for dependent children are to be paid by the parent who brings the child to the office for treatment, regardless of marital status.
